# Create your first form

This page walks through creating a form, adding fields, testing it and getting it onto your storefront.

## Step 1: Create the form

Open **Forms** and select **Create form**. On the **New form** page:

1. Optionally enter a **Title**. Leave it empty to use the template's name; you can change it later.
2. Under **Start from a template**, pick one and select **Use this template**.

| Template | What it contains |
| --- | --- |
| Contact form | Name, email, subject, message and the privacy consent |
| Newsletter signup | Email and a marketing consent checkbox |
| Wholesale request | Company, contact details, VAT number and a message |
| Return request | Order number, email, reason and a message |
| Feedback | A 1 to 5 rating, a message and an optional email |
| Blank form | Start empty and add your own fields |

A template creates a real form right away and opens it in the editor. Its fields come from your field library: the default fields (for example Email) are reused, and fields that are specific to the template are created for you.

## Step 2: Add and arrange fields

In the editor, select **Add field** at the bottom of the left panel. The **Add to form** menu has four parts:

- **Common fields** - ready-made fields added in one click: Email, Name, First name, Last name, Phone, Subject, Message, Company, Order number, Website, Address, ZIP code, City, Country, Date of birth, Newsletter consent and Privacy consent.
- **Your fields** - every other field in your library, with a search.
- **Layout** - Heading, Paragraph, Image, Divider, Spacer and Page break (next step).
- **New custom field** - choose a **Type** and a **Label**, then **Create and add**. The field is saved to your library, so you can reuse it on other forms.

On an empty form, **Add your first field** offers the most common fields as big buttons, with **More fields** for the rest.

In the preview, click a field to edit it. Its toolbar has **Move up**, **Move down**, **Duplicate** and **Remove from form**; hover between two fields and **Add a field here** inserts one at that spot.

## The default fields and the privacy consent

The common fields are the app's default fields. They are always in your field library. You can edit them and take them off a form, but they cannot be deleted.

Two of them are consent checkboxes:

- **Privacy consent** is required and reads "I have read and agree to the privacy policy", with the words "privacy policy" linking to your store's own privacy policy, the one you manage under **Settings > Policies** in your Shopify admin. The link is part of the label, so the consent is one text: click the field to change the wording or the link. If your store has no privacy policy there, point the link at your own page. - **Newsletter consent** reads "Send me news and offers by email." and counts as email marketing consent: when a visitor checks it, the Form submitted trigger sends `Marketing consent: true`.

Whether your form needs a consent checkbox, and what it has to say, depends on the law that applies to your store. The default text is a starting point, not legal advice.

## Step 3: Set the form's texts

With no field selected, the left panel shows **Form settings**. On the **General** tab set the **Title**, the **Handle**, the **Description**, the **Button label** and what happens **After sending**. These texts, like the texts of your fields, can contain Liquid, for example to show your shop's name or greet a logged-in customer. See [Liquid and HTML in your texts](https://docs.workflow-forms.app/liquid-and-html-in-texts.md).

## Step 4: Test and save

Select **Test like a visitor** above the preview and fill the form in. Conditions, steps and checks work as on your store, and nothing is sent or stored.

Then select **Save** in the save bar. One save stores the form, its fields and everything you changed on them.

## Step 5: Put it on your storefront

Above the preview, **Show on your store as** decides how the form appears:

- **Inline** - add the **Form** block to a page in the theme editor. See [Add a form to your theme](https://docs.workflow-forms.app/add-a-form-to-your-theme.md).
- **Popup** - the form opens over the page. See [Popups](https://docs.workflow-forms.app/popups.md).
- **Link page** - the form gets its own page with a link you can share. See [Direct link page](https://docs.workflow-forms.app/direct-link-page.md).

## Step 6: Act on submissions

Each submission lands in **Submissions** and starts the **Form submitted** trigger in Shopify Flow. Build a workflow on it to email your team, tag the customer or send the data elsewhere. See [Shopify Flow triggers](https://docs.workflow-forms.app/shopify-flow-triggers.md).
